MT. CARMEL REGIONAL MEDICAL CENTER NOTICE TO APPLICANTS

Mt. Carmel Regional Medical Center is firmly committed to maintaining a drug-free workplace and providing a safe environment for our patients and staff. Consistent with that objective, we require all applicants accepted for employment to pass an alcohol and drug test in addition to a health assessment; an investigative background check which includes a review of criminal records; information from past employers, educational institutions and any public or private agencies that have issued either a professional or vocational certification or license; a Medicare sanction review; and an orientation program as part of our employment process. Please be advised that all offers of employment are contingent upon satisfactory results of a drug-screening test; a health assessment including tests for TB, Rubella, Rubeola, a CBC; an investigative background check as previously described; and completion of the new employee orientation program. Applicants who refuse to sign the following Agreement will not be considered for employment. Those applicants who test positive for drug usage will be denied employment at Mt. Carmel Regional Medical Center.

DRUG SCREENING, HEALTH ASSESSMENT, BACKGROUND CHECK AND
ORIENTATION AGREEMENT

I UNDERSTAND AND AGREE THAT ANY OFFER OF EMPLOYMENT MADE TO ME BY Mt. Carmel Regional Medical Center is contingent upon the outcome of drug testing, a health assessment, an investigative background check as described above and completion of the new employee orientation program.

I understand that if I fail to pass the drug screen, e.g., if my urine specimen is positive for controlled substances or if my urine specimen shows any evidence of adulteration or substitution, I will be disqualified from further employment consideration with Mt. Carmel Regional Medical Center. I have provided complete and truthful information to Mt. Carmel Regional Medical Center regarding all sources of information about my past employment, education, licensure, certification, criminal conviction record, driving record (if required), as well as any other information requested in the employment application, and am aware that any misrepresentations or material omissions concerning such information will be grounds for denying my application, withdrawing any offer of employment, or immediate discharge, regardless of when such misrepresentation or material omission is discovered.
